Promotion Terms and Conditions
1. Only select doctors in the United States and Canada who have patients that have received prior direct to consumer (“DTC”) treatment with clear aligners shipped directly to the patient and who are seeking re-treatment with Invisalign following a consultation with their doctor are eligible to participate in this Promotion. Treatment with clear aligners fabricated by a laboratory pursuant to a doctor’s prescription are excluded from this promotion.
2. The promotion period runs from November 4, 2019 – December 31, 2019
. The patient must have received prior DTC treatment within the previous 12 months from the date of case submission under this Promotion.
3. Eligible re-treatments include treatment with Invisalign System – Comprehensive, Invisalign System – Moderate, Invisalign System – Lite, Invisalign Go and Invisalign Express.
4. Not all cases qualify clinically for re-treatment. It is the Doctor’s responsibility to offer re-treatment with Invisalign only where clinically appropriate.
5. Subject to compliance with these terms, the Doctor’s lab fee, after application of additional discounts, will be discounted by 50%. Doctor must pass the value of the lab fee discount to the patient seeking re-treatment with Invisalign.
6. In order to receive this promotional discount, Doctor must, prior to CCA acceptance:
a. Provide the following:
i. Patient’s name (which must match the Invisalign Patient ID);
ii. Evidence of prior DTC treatment (may include one of the following):
1. Proof of payment of prior DTC treatment;
2. Consent to prior DTC treatment; or
3. Image of prior DTC aligner packaged (if all required items are visible in the image)
b. Obtain and submit the patient’s completed Patient Consent and Release of PHI form.
c. Provide all required information reasonably required by Align regarding the re-treatment with Invisalign clear aligner therapy, including but not limited to the submission of before/after photos and related clinical information.
7. Promotion limits:
a. Subject to the restrictions and requirements herein, discount is applied per patient re-treated with Invisalign clear aligner therapy.
b. The total number of treatments accepted under this promotional program are limited to 1000 total submitted cases under this Promotion during the promotional period. Participants will be informed as soon as the enrollment limit is reached.
8. Doctor has responsibility to adhere to all laws, regulations and reporting obligations. Doctor has responsibility to report the discount under the promotion to any federal, state or provincial healthcare program that may provide reimbursement.
9. This Promotion can be combined with their offer or promotional program from Align, including those that provide upfront and/or volume pricing discounts. Specifically discounts under the Advantage Program, the Advantage Bonus Program and the Reingage program may be combined.
10. Notwithstanding the terms of the Group Discount Program Agreement between your organization and Align which states “doctors within Dental Service Organization will no longer be able to participate in any other Align discount program or promotional offer,” any such doctors within Dental Services Organizations are eligible to participate in this promotion.
11. Each Invisalign treatment submitted under this Promotion is subject to the Align Technology Pricing, Terms & Conditions. To review the Align Technology Pricing, Terms & Conditions, please visit the Invisalign doctor Site (IDS).
12. The contents of the Promotion are considered Confidential Information of Align Technology, Inc. and must be protected as such and may not be disclosed to any third party without Align Technology, Inc.’s written consent.
13. Any participating doctor who is not in good standing with Align Technology, Inc. as solely determined by Align Technology, Inc., is ineligible for this promotion.
14. By participating you represent that your participation does not violate any third party rights or policies, including those of your employer, any university, or research institution.
15. Doctor has responsibility to adhere to all laws, regulations and reporting obligations, including reporting the discount under the Promotion to any federal or state healthcare program that may provide reimbursement.
16. Prices, specifications, availability and terms of Promotion may change without notice. Align reserves the right to terminate this Promotion at any time without notice. All decisions by Align or its Promotion administrator are final.
17. Final invoice is inclusive of all discounts.
18. Submitting false claims may make you ineligible for future participation in promotional programs and may subject you to liability.
19. Align is not responsible for late, lost, stolen, misdirected, illegible, incomplete claims/requests or for any printing, human, computer or other errors. Nonconforming submissions will not be processed.
20. Align reserves the right to request additional information from you at its discretion.

Direct to Consumer Re-treatment with Invisalign | Promotion Period
Dr Patti Panucci attended the University of Louisville School of Dentistry for four years, where she graduated with a DMD degree (May 2000) among the Top 10 in her class. Following that, she headed west to Los Angeles to complete her three-year residency at one of the top-ranked orthodontic programs in the country – the University of Southern California.
Along with her certificate in orthodontics, Dr. Panucci earned a master’s degree in craniofacial biology. During those three years, she fell in love with Southern California beach life and decided that this was where her future lay.
